The undersigned is directed to refer to the subject cited above and to state that in future all the cases on the subject may be forwarded to the Administrative Department each and separate of an employee alongwith the following information documents to proceed further in the matter:-

  • The request of the widow;
  • Copy of CNIC of the deceased and the widow/ other concerned person;
  • Copy of the affidavit making it clear that the applicant is the legal hire/ head of the family of deceased;
  • List of family members;
  • Copy of the death certificate of the deceased official;
  • Copy of the last pay slip;
  • Copy of succession certificate;
  • Copy of service book.

2.  The above said information may be treated as check list. Combined case of two or more officials may be avoided to forward the Administrative Department through 2 single forwarding letter. Rather each and every case may be submitted to the Department separately.
